Well mine just happened. Our first of I'm sure many accidents to come. 

I was causally doing my business on the toilet when babe was playing in the hallway. He disappeared into my room which is baby proofed each morning! So I wasn't too concerned. In a matter of seconds I heard a bang and before he even started crying I was there and thinking the worst!

"my babe will never walk" 

He had pulled one of the bedside tables down ontop on himself! I threw it off him and he rolled over and began to cry. I was so relieved to see him moving his legs as I was thinking the worst.
